You must agree to the following terms and conditions to participate in the Virtual Community Groups for .

1. You are participating in the Virtual Community Groups voluntarily.

   1. You may stop attending the Virtual Community Groups at any time.
   2. Violation of any of the terms of this Confidentiality Agreement, as deemed by Defense Health Agency or TriWest, shall be grounds for removal from the Virtual Community Groups.

2. Sharing of Information is also voluntary.

   1. The group format involves sharing information to help yourself and others in the Virtual Community Groups.
   2. You may choose to share as much or as little as you feel comfortable during each session.

   3. Any information shared by you and others is confidential.

      1. You shall not share this information outside of the Virtual Community Groups.

3. You acknowledge and understand that there are risks, benefits, and consequences associated with this Virtual Community Groups operating in a virtual environment, including but not limited to, disruption of transmission by technology failures, interruption and/or breaches of confidentiality by unauthorized persons.

4. You agree not to disclose any information to those outside of the Virtual Community Groups that may reasonably be used to identify another member of the Virtual Community Groups.

5. You agree not to permit the disclosure of any information to those outside of the Virtual Community Groups during the Virtual Community Groups sessions to the extent it is within your control.

6. You agree not to share the meeting link with anyone not authorized by this Registration and Confidentiality Agreement.

7. You agree not to record any voice conversations, videos, or create still images (e.g. take pictures) of any information shared in these Virtual Community Groups by the use of any recording device, application, or otherwise.

   1. If it is discovered that you have any such recordings, you will be asked to delete those recordings. Additionally, if you are discovered to begin, or have in your possession, any such recordings, you will be removed from participation in these Virtual Community Groups.

8. You acknowledge and agree that TriWest must adhere to professional and ethical guidelines related to confidentiality, as well as state and federal laws governing confidentiality and patient/beneficiary privacy.

9. You acknowledge and agree that TriWest reserves the right to make disclosures of any information gathered in these Virtual Community Groups in a manner which is consistent and/or otherwise required by applicable state and federal laws.

10. You agree to indemnify, defend, and hold harmless TriWest and the government, including, but not limited to Defense Health Agency and Department of Defense, from any and all claims, judgments, costs, liabilities, damages and expenses, including attorney's fees, whatsoever, arising from any cause whatsoever in connection with your participation in the Virtual Community Groups, including but not limited to, the misuse and/or misappropriation of your personal and/or protected health information shared during the Virtual Community Groups sessions by another Virtual Community Group participant or persons who obtain your information through another Virtual Community Groups participant.
